About Collaborative Care

Collaborative Care is a place-based planning program working closely with partners and the community to improve access to healthcare in Liverpool Plains. It is crucial to receive timely healthcare because this directly impacts a person’s health outcomes. Delays in receiving care, tests, or results can lead to harm, emotional distress, and potentially long-term health issues. Our community survey and interviews with local health professionals confirmed that this is an issue for the community.

We need to look for ways to empower people in the community to take more control of their health. We also need to empower the broader community to provide opportunities for preventive health programs that encourage healthy habits and reduce social isolation. And we need to support our local health professionals with a professional environment and new opportunities to use their skills.
