Help for people in Liverpool Plains to find healthcare services

We heard from the community that many people find it challenging to understand how the healthcare system works, which makes it hard to find the services they need. When you aren't sure what to do, it helps to have someone you can talk to who has that knowledge. That's why we are starting a Care Navigator service in Liverpool Plains in August 2026.

How a Care Navigator can help you

A Care Navigator is someone who can connect you with the health services that you need. They act as a guide, helping you find your way through the health system when it feels confusing or overwhelming. Think of it like having someone in your corner who knows the local services, understands how the system works, and can point you in the right direction.

A Care Navigator can help you:

  • Find the right health services for your needs, whether you need a GP, specialist, mental health support, aged‑care services, disability supports, or community programs.
  • Understand how the healthcare system works, explaining things in plain language so you know what’s available.
  • Connect you with services, helping with referrals, appointments, forms, and follow‑ups.
  • Connect you with additional supports such as transport services, social supports, or preventative health programs.
