How a Care Navigator can help you

We heard from the community that many people find it challenging to navigate the health system and find health information.

A Care Navigator is someone who helps you get the care you need. They act as a guide, helping you find your way through the health system when it feels confusing or overwhelming. Think of it like having someone in your corner who knows the local services, understands how the system works, and can point you in the right direction.

In Wee Waa, many health services are available monthly or fortnightly, or by appointment only. Some services aren't seen because they are delivered in the home. This can make it hard to know what services you can access.

A Care Navigator can help you:

  • Find the right health services for your needs, whether you need a GP, specialist, mental health support, aged‑care services, disability supports, or community programs.
  • Understand how the healthcare system works, explaining things in plain language so you know what’s available.
  • Connect you with services, helping with referrals, appointments, forms, and follow‑ups.
  • Connect you with additional supports such as transport services, social supports, or preventative health programs.
