Health & Wellbeing Small Grants

We are offering grants of $500 - $5000 to Leeton Shire community to deliver initiatives which improve the health and wellbeing of the community.

Collaborative Care is a partnership of organisations that want to give the local community access to the right services to help people get healthy and stay healthy. We ask the community and your health professionals what they think is important, and then look for solutions together.

About Collaborative Care

Collaborative Care is a place-based planning program working closely with partners and the community to improve access to healthcare in Leeton.

One of the long-standing problems people have in Leeton is reduced access to GP services. When you don’t have easy access to a local GP, you don’t have healthcare when you need it.

It is crucial to receive timely healthcare because this directly impacts a person’s health outcomes. Delays in receiving care, tests, or results can lead to harm, emotional distress, and potentially long-term health issues. Our community survey in Leeton and the interviews with health professionals confirm that this is an issue for a lot of people in the community.

This shows we need to look at ways of attracting more general practitioners to Leeton. We also need to look at ways to support other health professionals in Leeton to work to their full potential to support the community around them with timely healthcare. This includes nurses and nurse practitioners, allied health professionals, pharmacists, care navigators, and Aboriginal health workers.
