Project Foundations Oct - Nov 2025

Establishing the Collaborative Care Project Leeton working group to drive the project's direction.

Working Group Includes:

  • Rural Doctors Network - Community Project Officer, Project Lead, General Manager of Services.
  • Leeton Shire Council - General Manager, Councilor Representative
  • Leeton & District Local Aboriginal Land Council - Chief Executive Officer
  • Murrumbidgee Local Health District - Leeton Hospital Facility Manager, Community Care Manager, General Manager Rural Operations
  • Murrumbidgee Primary Health Network - Executive Integration & Partnerships, Senior Management Primary Engagement
  • Ministry of Health Regional Health Division- Principal Project Officer, Project Support Lead

Investigate Needs Feb - July 2025

Building foundations for the project based on evidence gathered from community surveys, practitioner interviews, health provider workshops, and statistical health and demographic data.

Collaborative Care Project Leeton conducted the following research since October 2024:

Community Engagement: Health Services Survey in April 2025

Practitioner Feedback: Interviews with local GPs, practice managers, and owners July - August 2025

Health Provider Engagement: Health Planning Workshop June 2025, with 20 local allied health professionals in attendance.

Statistical Data: Health Indicators Report, HeaDS UPP (GP indicators report), Census on Leeton demographics.

Mapping: Service mapping for existing local services and outreach programs available.

Prioritise Needs Aug - Sept 2025

After thematic analysis of all the 'Identifying Needs' data, the Collaborative Care Project Leeton came together for a Prioritisation Workshop in August 2025.

The workshop systematically went through all the identified needs for health care and services for Leeton and confirmed three high-need areas;

1) Care Coordination and Interagency Integration

2) Mental Health

3) Enhancing Primary Care with a Family and Children focus

Co-Design Solutions Oct 2025 - Jan 2026

Working on the priority focus areas, the Co-Design Solutions phase takes a microscope into the different opportunities that will create sufficient improvement and long-term positive effectiveness in these areas.

During this phase, Collaborative Care Project Leeton works with health agencies, subject matter experts, health & wellbeing programs, and community members to clearly define what will be valuable in enabling the right solutions to these priorities.

Implement Solutions Feb - Oct 2026

Creating a project management plan for each solution to ensure the initiatives developed through Co-Design are implemented effectively and efficiently, with all stakeholders clearly understanding and fulfilling their respective roles.

Reflect & Learn Sept - Oct 2026

Take time to reflect on what aspects of the project are currently working well, identifying the successful strategies, processes, and partnerships that should be maintained or built upon. At the same time, examine areas where challenges remain or outcomes are falling short of expectations.

This balanced review helps to recognise achievements, celebrate progress, and inform continuous improvement by highlighting opportunities to refine approaches, strengthen collaboration, and enhance overall impact.
