Think of it like having someone in your corner who knows the local services, understands how the system works, and can point you in the right direction without all the stress.

A Care Navigator can help you:

  • Find the right health or medical services for your needs. Whether you need a GP, specialist, mental health support, aged‑care services, disability supports, or community programs.
  • Understand how the healthcare system works. Explaining things in plain language so you know what’s available and what might suit your situation.
  • Connect you with services. Helping with referrals, appointments, forms, and follow‑ups.
  • Connect with additional supports such as transport services, social supports, or preventative health programs.

Why Care Navigation is important

We heard from the community that a lot of people find it challenging to navigate the health system and find health information.

In Wee Waa many of the services available in town are outreach services. They are available monthly or fortnightly and are delivered by services based in other areas. This makes finding accurate and up to date information challenging. Accessing health services may involve navigating complex referral pathways, service eligibility requirements, telehealth systems, or community transport services.
