Meet Chelsea, Liverpool Plains Care Navigator

Liverpool Plains now has a Care Navigator! Chelsea Doran can help community members understand their healthcare options and the services available.

Here, she tells us a little more about her role and how she can support you.

Tell us a little about yourself and your role as Care Navigator.

My name is Chelsea Doran and I am the Care Navigator for the Liverpool Plains. I lived in the Liverpool Plains community for just over 7 years before recently relocating to Gunnedah. My role as a Care Navigator is to provide the community with accurate information to help people understand and access healthcare services.

What is a Care Navigator and how can you help people in Liverpool Plains?

Care Navigators help community members find their way through the healthcare system. I can help people understand what services are available and how to access them.

What are some reasons people might get in touch with you?

I can assist with booking medical appointments, filling in medical forms, finding the right health services and connecting people with local supports like transport.

Do people need an appointment or referral to access the service?

No! You do not need a referral or an appointment to access the service. On the days I am in Quirindi or Werris Creek, you can walk in and see me.

What can people expect when they meet with you?

A supportive, compassionate and confidential conversation. II take time to listen, understand each person’s circumstances and work with them to access the right services and supports

What would you like people in the community to know about the service?

That this is a free, confidential service and I am available Monday-Friday even when I am not on site. I can support people of all ages in the community.

Chelsea will be at the Council Chambers in Quirindi every Wednesday from 9:30am to 3:30pm and in Werris Creek Library every Friday from 10am to 3pm. A sign will be displayed on site when she is available.
