Being Supported Through Your Healthcare Journey

Healthcare needs can be complicated. A person may need help from doctors, nurses, pharmacies, other health workers, and community services. When these services work together, the person gets safer, faster, and better care.

For example, a person managing diabetes may see:

  • a family doctor
  • a diabetes specialist
  • a dietitian
  • a pharmacist
  • an exercise physiologist
  • and a podiatrist for foot care.

Connected healthcare services are important because they help people get the right care, at the right time, from the right people. And they can explain your next steps clearly to you. Well-connected healthcare services can help people feel more supported and recover from illness more quickly. The person does not need to explain their health story over and over again to get the care they need.

This can be a challenge in towns like Eden, where some health professionals only visit occasionally or are located in neighbouring towns. We have to plan how to link different health teams together so people get the care they need.

How Are We Achieving This?

Lots of different organisations offer healthcare to the Eden community. Each organisation offers important health services, but the community will receive the most benefit if these services are coordinated in partnership with others. For example, several new care navigation services will soon be available to the Eden community that will help people understand the help they can access. The three different organisations that are offering this new service will work together, so it is a simple change that is easily understood and accessed by residents.

Some health services in Eden may have the potential to relocate to a more suitable location, potentially co-located with other health services. This would make these services easier to access and provide more opportunities for different health teams to work together. Over the next few years, we will review the location of services such as Community Health and look for alternative options in Eden that may offer more to the community.
