Diabetes Shared Care Clinic
If you live in Wee Waa, we can help you manage your diabetes. Our Diabetes Shared Care program is designed to suit your needs and help to coordinate your care.
The HealthWISE care manager will work with other health professionals to help you follow your GP Management Plan and Diabetes Care Plan.
Who Can Access Services?
A person living in either Narrabri or Moree Local Government Areas (LGAs) with diabetes, including: Type 1, Type 2 and Latent Onset Diabetes in Adults (LADA).
You will need to talk to your GP for a HealthWISE referral.
