The Snowy Valleys Project addressed access to primary health care, which encompasses general practitioners, allied health professionals, primary care nurses, pharmacists, and Aboriginal and Torres Strait Islander health workers. The project strengthened relationships between local health providers and provided an avenue for renewed communication and collaboration.
The project also trialled Shared Medical Appointment (SMA) models in the Snowy Valleys and the neighbouring Riverina area. These trials delivered medical appointments in a group setting for patients with the same chronic health condition. The trials showed a range of benefits for both the patients and the health providers that are not possible in the traditional model of general practice (1:1 appointments).
