Since 2018, a local GP in Canowindra NSW, has been trialling a deliberate team-based care model (DTBC) for patients with a greater risk of health decline. These patients are case managed by a multidisciplinary team representing general practice, specialists, pharmacy, paramedicine, nursing, allied health, and aged care.
The Collaborative Care Program evaluated the lessons learned from the model, including the benefits and challenges of a team-based approach to primary health care in this rural community.
The study found that deliberate team-based care provides a range of benefits to patients with ongoing health conditions. This primary care model was effective in reducing the number of hospital stays for patients with intensive healthcare needs.
